چکیده

Alpha-1 antitrypsin deficiency (AATD) is a hereditary disorder characterised by low circulating levels of the key antiprotease alpha-1 antitrypsin (AAT) and is associated with the development of chronic obstructive pulmonary disease (COPD), often by the 3rd or 4th decade, and liver disease. The two most common SERPINA1 mutations associated with AATD are the Z and S mutations, and the vast majority of AATD individuals diagnosed with COPD are ZZ homozygotes. AATD is an under-diagnosed condition with the majority of cases misdiagnosed as COPD. The World Health Organisation (WHO) and the American Thoracic Society/European Respiratory Society (ATS/ERS) advocate a targeted screening approach for the detection of AATD in patients with COPD, non-responsive asthma, cryptogenic liver disease and first degree relatives of known AATD patients (Alpha 1antitrypsin deficiency: memorandum from a WHO meeting 1997; American Thoracic Society/European Respiratory Society statement: standards for the diagnosis and management of individuals with alpha-1 antitrypsin deficiency. 2003). It is our contention that a diagnosis of AATD gives the clinician a vital and unique opportunity for early medical intervention and the possible prevention of COPD in both the affected individual and first-degree relatives. Unfortunately, despite huge strides in awareness and understanding of this condition, this opportunity is too often missed.

Background: One of the genetic risk factors for chronic obstructive pulmonary disease (COPD) is deficiency of Alpha-1 Antitrypsin (A1AT). There is no exact statistics about the prevalence of this disease in different regions of Iran. The present study aimed to determine the prevalence of alpha-1 antitrypsin (A1AT) deficiency in COPD patients in Kerman, Iran. Metho...

Estimating the risk for alpha-1 antitrypsin deficiency among COPD patients: evidence supporting targeted screening.

Alpha-1 antitrypsin deficiency is known as a significant genetic risk factor for COPD for carriers of phenotype PIMZ, and for phenotypes PIZZ and PISZ. Genetic epidemiological studies for alpha-1 antitrypsin deficiency conducted by others on both COPD patients and concurrent non-COPD controls were used to estimate the risk factors for all six phenotypic classes (namely, the normal phenotype PIM...

Assessment of Alpha-1 Antitrypsin Deficiency in Patients with Severe Chronic Obstructive Pulmonary Disease

Background and Aims: Chronic obstructive pulmonary disease (COPD) is a kind of pulmonary diseases characterized by chronic obstruction of lung that is in the form of a diffuse narrowing of airways resulting in air flow resistance. Alpha-1 antitrypsin (AAT) deficiency is genetically relatively common risk factor in patients with COPD throughout the world and the exact cause of its prevalence is ...

Alpha-1 antitrypsin deficiency: whom to test, whom to treat?

Alpha-1 antitrypsin deficiency (AATD) is the major identified genetic risk factor for chronic obstructive pulmonary disease (COPD). The biochemical events leading to lung destruction in AATD are well understood, and most of our understanding of the pathogenesis of COPD in general has been acquired through the study of AATD. There is a growing appreciation that early diagnosis of AATD can affect...

Alpha-1 Antitrypsin Deficiency in Iranian Patients with Chronic Obstructive Pulmonary Disease

BACKGROUND Alpha-1 antitrypsin deficiency is a genetic disease which affects both lung and liver. This disease is a recognized factor for chronic obstructive pulmonary disease (COPD). However its importance as the cause of COPD in a country such as Iran is unclear. OBJECTIVES This study was conducted to find out the role of α-1 antitrypsin deficiency as a cause of COPD in Iranian patients. ...
